A three-session introduction designed to complete a comprehensive assessment and develop your individual rehabilitation program.
Your package includes:
An initial assessment and goal setting
Development of your individual rehabilitation program
Two follow-up sessions to refine and progress your program
Ongoing Rehabilitation

Home Visits
Initial Consultation
Standard Assessment
$190 · 60 minutes
A comprehensive initial assessment to understand your history and goals, assess your physical and functional rehabilitation needs, and establish your individual rehabilitation plan.
Comprehensive Assessment
$260 · 90 minutes
An extended initial assessment for clients with more complex rehabilitation needs.
Ongoing Rehabilitation
Standard Session
$180 · 60 minutes
Extended Session
$250 · 90 minutes
Travel
Travel is included for home visits within the Eastern Suburbs.
Funding & Rebates
Medicare rebates may be available for eligible clients referred under a GP Chronic Condition Management Plan.
Private health fund rebates may also be available depending on your insurer and level of cover. Please contact your health fund directly to confirm your eligibility and rebate.
Services may also be available for clients accessing funding through the NDIS, My Aged Care and SIRA, depending on individual funding arrangements and eligibility.

Cancellations
Please provide at least 24 hours notice if you need to cancel or reschedule your appointment.
Cancellations or changes made within 24 hours of the start time will be charged the full appointment fee.
